Assertive Community Treatment Team (ACTT)

ACT Teams provide an array of support services that may be the key to living full and satisfying lives in the community. Clients who benefit most often have a psychotic disorder while some also experience other diagnoses or complex situations.
ACTT is a client-centred, recovery-oriented model guided by provincial standards. It is tailored to the needs of those who typically experience frequent or lengthy hospital stays or use of mental health services, but who have not benefited from the traditional approaches. ACTT’s multidisciplinary team provides a range of comprehensive treatment, rehabilitation and support services to a limited number of clients. Teams typically include a psychiatrist, nurses, mental health workers, occupational therapists, peer specialist and other workers.
There are many ACT Teams across Ontario and beyond. CMHA Cochrane-Timiskaming Branch has two teams: one supporting people living in Timmins and one for those residing throughout the District of Timiskaming.
Our ACT clients benefit from continuous and time unlimited access to:
- Symptom treatment including regular visits with a psychiatrist and assistance managing medication if necessary;
- Primary care services;
- Crisis support available 24 hours a day;
- Help dealing with substance abuse;
- Work-related services;
- Assistance with activities of daily living;
- Social and leisure-time support;
- Family-centred service; and
- Advocacy.
Our ACT Teams work with a small number of clients, primarily in community locations where clients are living their lives, and the services are available around the clock and on an on-going basis as long as the services are wanted and needed. Service is personalized to address each clients’ unique needs, preference and goals.
