Having access to primary care is essential for anyone’s health. Living with a mental illness can bring additional risks to a person’s health. Through the team work of our nurses, nurse practitioners, and collaborative physicians, we provide services to individuals living with mental illness who are without a family physician and give them an equitable chance of better health.
We provide comprehensive primary health care to respond to the needs of the whole person and ensure continuity of care, acute and chronic disease management, as well as health promotion and disease prevention. Referrals and coordination with other levels of care internally or externally are provided when required. The ongoing care is enhanced, as the primary care providers work closely with our mental health teams so they can support clients as necessary in understanding, making decisions, and following through with their healthcare in a client-centered approach.
